I understand and agree that the employees, officers, directors and agents of Doggy Day Care, Inc will exercise due care and diligence in the care and safety of my dog while in their care. In the event that my dog becomes injured or ill, I hereby give permission to Doggy Day Care, Inc to seek whatever medical attention is necessary to treat my dog and I agree to pay all charges incurred in the treatment of my dog. I hereby consent to the veterinarian of Doggy Day Care, Inc's choice to be retained for the treatment of my dog.

I understand and agree that while under the care of Doggy Day Care, Inc, my dog will be interacting with other dogs and people and understand that there can be a chance of injury while interacting with other dogs and people and understand Doggy Day Care, Inc will take all necessary steps to ensure the safety and well being of my dog in its care and I assume all risk of injury or illness to my dog while at Doggy Day Care, Inc or while being transported to any veterinarian for treatment of such injury or illness.

I hereby release Doggy Day Care, Inc, its employees, officers, directors and agents from any and all liability of any nature for injury, illness or death of my dog resulting from Doggy Day Care, Inc's actions or the actions of my dog or any other dog while in the care of Doggy Day Care, Inc or on the grounds of Doggy Day Care, Inc except for actions resulting in negligence by Doggy Day Care, Inc.

In the event that my dog causes injury or death to another dog or to a person while in the care of or on the grounds of Doggy Day Care, Inc, I indemnify and hold harmless Doggy Day Care, Inc from any action which may be brought against it, and for any defense, settlement or judgment entered against it resulting from such action of my dog. I also agree that if my dog causes injury to another dog or to a person while in the care of and on the grounds of Doggy Day Care, Inc, I agree to pay the veterinary and/or medical expenses of the injured dog and/or person. I will assume all liability for the actions of my dog.

No pet will be released until all charges are paid in full or other arrangements satisfactory to our kennel have been made. The customer agrees to notify us in advance if there is any change in the pet's pick up date. Any pet left uncalled for seven days from the scheduled date of pickup, as noted herein, shall become the property of Doggy Day Care, Inc. The owner of this pet agrees to pay reasonable legal fees and costs incurred by the kennel in the collection of outstanding bills.
